Information submitted through this form is analyzed in aggregate to identify trends related to fertility treatment outcomes. We do not request names, date of birth, medical record numbers, or direct identifiers. Submitted data is processed in a de-identified or aggregated format and may be used for reporting, analytics, and research purposes. 

​IF YOU HAVE MORE THAN ONE EMBRYO TRANSFERRED AT ONCE, PLEASE COMPLETE ONE FORM FOR EACH EMBRYO TRANSFERRED.​​
